I was looking through my coins

Hey so I  was looking through my coins and I found this. I would like to know if this was done intentionally or if it was done at the mint and if it was done before circulati, how would I know how much more that it’s worth?

Clear photos in the proper orientation would be helpful. This is clearly PMD (post mint damage) and worth exactly 25 cents. Except in a vending machine where it  will probably be rejected.
