The reverse is a close match to this one: N#159260

There are multiples variations.

You can see a quite different lettering  for other 3 lines tokens: 

yours is : “Sultan of the 2 lands and 2 seas” (translation given by Numista - I don't read arab)

وخاقان البحرين

Then on Numista coins it's : "Mahmoud Khan" 

محمود خان

So it's a very different token/medal and you should enter it in the database along with other ones
