King George IV or III Medal. 

The medal is bronze or copper and is 22mm.

 The one I found here at Numista is 25mm. 

Is it a variant?

Noonans didn't actually specify the diameter of the medal in their auction, as it was an additional add-on to the main items in the lot. Hence I think 25mm may be a guestimate, and probably the same size as yours.
