This is not located in the Catalog, but need more info if possible.

46.5mm,

21.53g,

Medal alignment,

Two casting sprues on reverse bottom.

Can be Vietnamese: https://en.numista.com/catalogue/index.php?r=&st=147-150-149&cat=y&im1=&im2=&ru=4369&ie=&ca=3&no=&v=&a=&dg=&i=&b=&m=np&f=hole&t=&t2=&w=&mt=&u=&g=&c=&wi=&sw=

But the reverse doesn't match, the size is way too large, and the coin itself is too crude. My guess is that it's some sort of charm or token, based on the linked coins.

The closest I found is:

明命通宝背君臣父子 : reverse side depicting the emperor, ministers, father, and son.

(Emperor on top of reverse)

Referee of south atlantic islands

This type has a number of examples cataloged on Zeno, it’s a Vietnam War era fake/charm coin:
