Change request - report of the reason of modification

Is it possible that in mine "Change requests" the reason of the modification could be displayed?
You have to put in the reason for that modification, but that reason isn't displayed.

I mean if you click on a request, you see what the suggested modification(s) are, but not the reason why I suggest to make that modification.

I can see the changes too I made on july,1-2013 and been accepted just a couple of days ago. So both changes are made!

Another question : is it possible to see the date of that accept/reject? That day I could not see at this moment.

So I suggest : REQUEST STATE: ..........  ON .{date of request-state}.
