Carolus IV, 1791 coin, help. Any info, thoughts

I normally collect Greek and Roman coins. This was a gift and I think it's not real... Any thoughts

I'm completely lost here and have no idea about the countermark and solder mark on the middle.....
Looks like it is cast?
That could explain the two dots on the coin.
I have no idea what the countermark means.
Mint looks like Chili (S)

I could not find a coin with CAROLUS IV.
Most of the coins have CAROLUS IIII....

Yes, I suppose that there are people who collect fakes and/or replicas. I actually have a friend who collects replicas made in antiquity, very old counterfeit coins... Some very well made.

This was just a curiosity for me....
