The text in the exergue on the obverse says that the medal commemorates the death of the former King of Sardinia-Piedmont, Carlo Alberto in exile in Porto, in 1849, after he abdicated following the failed First War of Italian Unification with Austria. That explains why it's in Portuguese.
Thank you CassTaylor, didn't know he was in Exile in Portugal. Other side is marked with 1854 in roman numerals so could this medal commemorate remembering 5 years of his death?
