How should I call it? Comune of Piacenza or just Piacenza? Some Italian states are done just that way, simply. Also, what differs it from duchy of Piacenza?

It should be called Comune of Piacenza. In Italy comunes were like free cities, there were some local governing system, it's similar to republic. When Piacenza became duchy, the rule of local government was finnished and the power went to the Duke and his House.
