It doesn't look like the original so it may be a modern copy of this medal

A modern imitation of a Giovanni dal Cavino medal which in itself was a imitation of ancient Roman coins/medals. You can find multiple examples here:

It is definitely a medal (Limbru provided you a link), but it is not necessarily a fake just a copy of the original medal that has been struck by Giovanni da Cavino.
